The Employee Performance Evaluation Form can be used during an employee’s probationary period. This form will gather information based on their performance in general as well as in relation to their specific job. The form will pinpoint areas needing improvement as well as show the areas in which they excel. A detailed comment area is available to document the reasons for each rating and also for the employee to make comments regarding the evaluation. In addition to this form we also offer the Performance Improvement Plan which is used as a positive written plan of action to guide employees back to the set expectation level.
