Legal authorization is necessary prior to depositing funds directly into your employees’ account. This form is set up to acquire the banking information for either checking or savings along with a signature approval. The employee will have the option of disbursing the net amount into one account or splitting the amount between two accounts. This form should be filled out by each employee should they choose to have direct deposit.
