Accidents and injuries that occur at work must be recorded within 7 calendar days of the incident. OSHA requires this documentation to be kept on file for five full years following the incident. The Accident/Injury & Illness Report complies with OSHA 301 form and requires information not only regarding the incident but also requests that health care provider information be recorded. All treatment whether onsite or at a facility must be logged including any hospital stay. This report can conveniently be stored in the employee file once the incident has been evaluated.

  • 05/2023 - Fair Labor Standards Act (FLSA) - Federal Minimum Wage - Revised the Nursing Mothers section of the poster with the PUMP for Nursing Mothers Act (PUMP Act) and more clarification on employer requirements to provide reasonable break time and accommodations for nursing employees.
