  • Workers' Compensation – Enter the name of your insurance carrier and the ID number.
  • Emergency Information – Pertinent emergency information including ambulance, hospital, fire/rescue, physician, police and CAL/OSHA contact.
  • Access to Medical and Exposure Records By CAL/OSHA Regulation – Fill in who can obtain the files and where the files are located.
  • Payday Notice – Enter the name of the company, frequency of payment, and the name of the person who will handle inquiries regarding paydays and their title.

  • Revised 04/2014 – CALOSHA – Updated with “Safety Data Sheets” (SDS) information to replace “Material Safety Data Sheets” (MSDS).
  • Revised 12/2013 – California Minimum Wage – Updated to reflect minimum wage increases for 2014 and 2016. The 2014 minimum wage is $9.00, effective July 1, 2014.
  • Revised 12/2013 – Discrimination Notice – Update includes the addition of military or veteran status as protected classes.
  • Revised 12/2013 – Unemployment Insurance – Update includes physician/practioner instead of doctor's office as an option for claiming disability insurance.
  • Revised 12/2013 – Withholding Allowance Notice to Employees – Update to Employee’s Witholding Allowance that has new classification for “Personal income Tax”.
  • Revised 11/2013 – CALOSHA – Updated to list Orange County under Area & Field offices.
