The Massachusetts Department of Workforce Development - a division of Unemployment Assistance - requires that all employers provide their employees with detailed information about their unemployment insurance benefits. According to the Employment and Training Law, each employer must provide employees with this pamphlet if the employee is out of work either permanently or temporarily for 7 or more days.

Employees that have lost work can find information about how to file for unemployment benefits, how to find jobs, eligibility, and where to file claims by using this form. The step-by-step instructions make filing for a claim easy to understand. Also, the form comes in several languages so that all employees can understand their benefits and how to file for a claim.
