OSHA requires that every employer post this summary in a prominent place from February 1 to April 30 of each year. Employers are subject to heavy fines unless they keep detailed records of any work related accident that results in death, loss of consciousness, time off work, being transferred to “light duty,” or medical treatment beyond simple first aid.

Accidents and injuries that occur at work must be recorded within 7 calendar days of the incident. OSHA requires this documentation to be kept on file for five full years following the incident. The Accident/Injury & illness Report complies with OSHA 301 form and requires information not only regarding the incident but also requests that health care provider information be recorded.
