The OSHA Log Series Booklet is designed to make recordkeeping easier. It includes detailed directions on filling out the OSHA form 301 Injury Report and OSHA 300 and 300A forms. It is always advisable to have the booklet and OSHA forms readily available at a moment’s notice. The bright yellow cover makes it impossible to misplace, even in the busiest office.
The Form 300A meets all OSHA requirements for a summary of work-related illnesses and accidents, including the updated recording criteria for occupational hearing loss. The form provides spaces for the number of accidents, type of incident, work days lost and all required signatures. By posting the Form 300A and filling out the form properly, you would complete your mandatory record keeping requirement.
Accidents and injuries that occur at work must be recorded within 7 calendar days of the incident. OSHA requires this documentation to be kept on file for five full years following the incident. The Accident/Injury & illness Report complies with OSHA 301 form and requires information not only regarding the incident but also requests that health care provider information be recorded. All treatment whether onsite or at a facility must be logged including any hospital stay. This report can conveniently be stored in the employee file once the incident has been evaluated.
