The Accident/Injury & illness Report complies with OSHA 301 form and requires information not only regarding the incident but also requests that health care provider information be recorded. All treatment whether onsite or at a facility must be logged including any hospital stay. This report can conveniently be stored in the employee file once the incident has been evaluated.
