A health care provider who has submitted the required bills and medical reports to an insurer and who disputes the amount or timeliness of the payment made by a self-insured employer/insurer, may seek a review of the disputed fee by the Bureau of Workers’ Compensation by filing an Application for Fee Review (LIBC-507).
