Description The Hawaii Department of Labor requires that all employers complete a detailed First Report of Injury, form WC-1, when an employee wishes to receive worker's compensation. Accordingly, the worker's compensation form should be used whenever there has also been a work-related injury or illness that has caused an employee to be absent from work for one day or more and whenever medical services are required, including first aid. The Hawaii First Report of Injury, form WC-1, needs to be completed within 7 days of the injury to the employee. If the form is not reported immediately, the employer could face a $5,000 fine. Also, the Department of Labor needs to be notified immediately if death occurred because of a work-related injury. Obtain and have the forms readily available in case an injury occurs.
