The handout contains information about where employees can obtain more information about the Workers’ Compensation program, including a 24 hour safety hotline phone number. The notice also contains contact information for resources that an employee can use to file a Workers’ Compensation claim.

If an employee is injured, becomes ill, or dies in the workplace and wishes to file a Workers’ Compensation claim, the employer should make the necessary paperwork available to the employee. The employer should then complete its portion of the paperwork and file it with the Taxes Division of Workers’ Compensation.
