When 25 or more employees separate on the same day for the same reason, employers are to fill out two mandatory forms.  One is the Mass Separation Notice and the Mass Separation-List of Workers form.  Both forms are included in the Georgia Mass Separation Packet for you to comply.  When a mass separation such as this occurs, all forms must be completed and either faxed or mailed to the Georgia Department of Labor within 24 hours after the date of separation.
