Description The state of Oregon requires that employers complete the First Report of Injury or Illness Form in order for employees to file for workers' compensation. Whenever an employee is injured on the job, the employer must complete information about the employee, the type of injury, any machinery involved in the injury, what medical treatment was required and more - no matter how large or small the injury is. When there is an injury that results in an overnight hospital state, OSHA requires that the injury be reported within 24 hours. If the injury results in death or a major catastrophe, the situation needs to be reported within 8 hours. This form is required in order for employees to receive workers' compensation benefits and cannot be substituted with any other form.
