The Nevada Department of Employment, Training, and Rehabilitation Employment Security Division require that all employers provide employees with information about unemployment insurance. The form must be provided to each employee that is eligible for unemployment benefits under the law.

The form also describes which employees might not be eligible for benefits, such as employees that quit their last job, were discharged due to misconduct, or employees that give false information about their eligibility or wage earning status.
