Description Employers in the state of Minnesota are required to report each injury that is sustained on the job - regardless of how serious or minor the injury is. According to the Minnesota Department of Labor and Industry Workers' Compensation Division, this first report of injury needs to be completed within 14 days of the injury. If the claim involves death, then the report needs to be completed within 48 hours of the occurrence and can be reported initially by telephone. Obtain the Minnesota First Report of Injury Form to ensure you comply with regulations if an on the job injury occurs.
